Position Description:



Registered Nurse- telephone triage, part-time, hours, with opportunity to add shifts as they become available, evenings and weekends.



Exclusively RNs nurses work from remote locations across the United States, actively triaging calls from around the country. Calls are routed to registered nurses through our web-based phone system, each call is answered live by a registered nurse. Patient episodes are documented and transmitted electronically to offices.





Requirements are:

RN-Eng/Spanish-Telehealth Triage-Work from Home (CA)


Our client is a telehealth services company. Their goal is to provide telehealth monitoring services for chronic disease patients so they can continue to live independently and decrease patient visits to hospitals and doctors. The Telehealth Nurse will be monitoring patients with and without Telehealth equipment.



Responsibilities:

Responsible for triaging incoming calls from patient, clients, assessing needs, giving appropriate clinical dispositions, scheduling appointments and facilitating referrals to primary providers, specialists, healthcare facilities and community resources. This nurse will also conduct outbound calls to educate and coach the patient and assess present status, providing immediate care advice and preventive behaviors. Using nursing assessment skills and data, patients will be coached to remain in a safe range.

The Telehealth nurse will provide skilled nursing care to assigned telehealth patients including assessment, evaluation, development of an individualized plan of care, patient/caregiver education in accordance with the agency policies and procedures and the physician care plan. Coordinates clinical interventions with physicians and the clinical team. Provides supervision to family/caregivers involved in the care of the patient.





Skills/ Requirements

MINIMUM QUALIFICATIONS:



We need an RN with a current CA license who speaks fluent Spanish, an additional AZ license a plus.

At least 2 years experience as a registered nurse with chronic disease management or as a discharge nurse in a hospital within the last 4 years. Demonstrated excellence in customer service and strong verbal and written communication expertise required. Basic telehealth counseling skills, interview/communication skills, critical thinking and problem solving skills. Ability to work within approved procedure and clinical guidelines. Cardiac clinical experience required. 1 to 2 years telephone triage preferred. Must be positive, enthusiastic, helpful personality who is organized and able to set priorities. Must have computer experience with EMR software .

Must have home office. One week training in Scottsdale AZ.





20 hours Mon-Friday to start up to 40 later on.

    Nurse Writing Assistant

    I am working on a project to show a program to educate nurses on Bioidentical hormones. This is all fictitious...so you can use nursing creativity. I have created a lesson plan with pre and post-tests for six nurses leaning hormone replacement therapy. The lesson plan will be administered to six nurses within a health center over a three day period. Before two of the learning sessions the nurses take a pre-test and then after they take a post-test. The third learning session consists of direct observation of the nurses performing the tasks. I currently have a single running document with all of the accomplishments of my project thus far. Each section is divided into objective numbers...you will see it when you open the document! I want this to seem real and convincing! APA format for the writing sections is preferred, but not necessary. Please no plagiarism :). Please see below for the assistance I am needing.

    1. The running list of accomplished objectives need their own Word document. For example 1a., 1b., 1c.,ect. will each have their own separate word doc. The word doc will need titled as the objective.

    2. I need a mentor (you) review the pre and post-tests, observation checklist, and optional class survey. The mentor is reviewing to see if any changes should be made and also making comments as to if changes need made prior to administration to nurses. (150-200 word comment about the tests, surveys, and observation checklist.)

    3. A grading scale needs developed for the pre and post-tests.

    4. After the administration of the tests and observations for the three section(s) a summary and analysis of the results needs done. For example, how did the nurses do on the pre-tests, did their scores improve on the post-test...any analyzing you can do will be extremely helpful!! The six nurses have worked for the center for less than two years, but are familiar with the content being presented to them. (300-600 words)

    5. Next we pretend we provided the nurses with an optional survey during last educational session for the nurses to provide confidential feedback...then collect surveys and compile a summary and analysis of the nursing staffs’ additional comments related to the education of bio-identical hormones. So the summary is what I need help with here. (300- 600 words)

    6. Then I need some assistance with writing a summary of the "experience" presenting this educational program to the nurses!! (300-600) words

    I created a needs assessment to determine what education the nurses needed. Objective 1b.,1c.,1d., are examples as to how the mentor reviewed the needs assessment tool and my analysis after administering the needs assessment to the nurses.
